At Florida OrthoCare, we welcome new patients. Please download the attached new patient packet and be sure to bring the forms to your scheduled office visit.
Medical Information Requested in Order to Schedule an Appointment:
- Be able to describe the injury as best as you can to our appointment personnel so that they can schedule you with one of our physicians who specializes in that area.
- Have your insurance card available so the appointment personnel can enter the information.
- Check with your insurance carrier to make sure that care services are in your network of approved physicians.
- The appointment availability of the individual physicians varies. Typically, we can see an emergency within 24 to 48 hours. For all other appointments please call our scheduling department for current availability.
